United Airlines Medical Information Form (MEDIF)
United Airlines Medical Information Form (MEDIF)
Passengers with medical conditions not treated and stabilized, or passengers which conditions that can be aggravated during flight, must have medical clearance before flight reservation.
A MEDIF form is usually completed by the treating doctor to describe the passenger’s medical condition and any special travel needs, such as oxygen, wheelchair assistance, medical escort, or stretcher arrangements.
United Airlines may review the MEDIF before confirming whether the passenger is fit to fly and whether special assistance can be provided.
Important Travel Note
For oxygen, stretcher, wheelchair, or other medical assistance, contact United Airlines or your travel agent as early as possible before departure.
